As we age, our needs change — it's just part of life. At a point, we may find that certain activities are more difficult or limiting than they used to be. But not everyone is ready to leave the home they love, the community they know and the independence they've had their whole life. Other times, caregivers looking after a loved one begin to experience increased stress and realize it's time for a little extra support.
When the time comes for you or a loved one to find assistance, know that it is AtlantiCare 's mission to keep its participants living in the community. As a Program of All-Inclusive Care for the Elderly (PACE) in New Jersey, AtlantiCare  is a nursing home alternative that gives you a personalized team of health care professionals to provide services to you in a setting of your choosing — whether that's in your home, in our  Center or at one of our other facilities.
We come to you to help with care coordination, transportation, social services, meals and any other day-to-day things you may need assistance with — but none of the ones you don't. Our goal is to keep you healthy and independent.

LIFE (Living Independently For the Elderly) Connection takes the guesswork out of coordinating care. From setting up and following through with appointments to bringing any necessary medical equipment home to writing and filling prescriptions, we take care of everything. And we’re always assessing and evaluating your situation so that we can adapt and make changes to your care plan and team, adding or removing providers or treatments as needed to support you and your caregivers.

Coordinating multiple providers, navigating complex care needs, and frequently traveling to and from a nursing home can often overwhelm loved ones — and at times even strain relationships. But this style of older adult care services provides families the opportunity to spend their time together doing things they love, in the place where they’re most comfortable.

There still may be times when you’ll wish to be a part of your loved one’s care. So we keep you involved every step of the way, beginning by working directly with you at the initial consultation to determine if this style of care services for older adults is a good fit. Then we’ll decide together what resources are needed to support your loved one — and help alleviate some of the strain you may be experiencing.
